Job Description
The Center Case Manager primarily provides Casey Life Skills Assessments (CLSAs) and Independent Study Guides (ISGs) to current foster care youth between the ages of 14 and 18 years old, in a 13-county area
Responsibilities:
- Begins the CLSA and ISG completion process for referred youth within a five-day timeframe by making contact via telephone, email, or mail.
- Conducts and scores CLSAs, facilitating virtual discussions with youth, caregivers, and agency staff to review results, offer life skill activity recommendations, and share information on HAY Center services like Transition Coaching, Summer Jobs Program, and College Tours.
- Assists with ISG completion for youth in placements where no support is available, serving as an alternative to classroom settings.
- Prepares and submits weekly/monthly reports to the direct supervisor.
- Enters all required documentation into a case management database, maintains case files, and supports in-person Life Skills Training when needed.
Qualifications and Requirements:
- Bachelor's degree in Education, Psychology, Sociology, Social Work, or a related field
- At least one year of full-time, paid experience working with adolescents, focusing on group facilitation in classroom, educational, or social service environments.
- Sufficient knowledge or experience in working with youth that may be aggressive or in crisis.
- Knowledge and understanding of adolescent/transition age youth stages of development.

In the spirit of pay transparency, we want to share the base hourly range for this position is $20 – $22 not including benefits, potential bonuses or additional compensation. If you are hired, your base salary will be determined based on factors such as individual skills, qualifications, experience, and geographic location. In addition, we also believe in the importance of pay equity and consider the internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is the full base salary range for the role. Hiring at the maximum of the range would not be typical in order to allow for future & continued salary growth
